"Fall Arrest System failed - worker injured"

On December 12, 1997 a worker fell 33ft and suffered serious injuries when one of the components of his fall arrest system failed. The victim escaped death only because he landed in a foot deep mud with a thin layer of ice on top. An investigation showed that the failed FAS component was severely worn out and should have been removed from service. Several other similar components were found to be in the same condition at this employer. No equipment inspection program was in existence at this facility. More importantly, there was a serious misconception among the users as to the component itself and what was a critical factor to look for when inspecting it.

The above accident was 100% preventable had the employer had at least one person trained to inspect the equipment. The seminar below was designed with this type of training in mind.



QUALIFIED INSPECTOR OF FALL PROTECTION EQUIPMENT

Two-day Hands-On Training Seminar


This Seminar is of special interest to companies with employees who use fall protection equipment. Some very common types of this equipment, the self-retracting devices (a.k.a. retractable lifelines, safety blocks) which complied with the CSA Z259.2-79 Standard, in most cases, do not comply automatically with the Z259.2.2-98 (R2004) Standard that replaced the old one. The participants will be shown how, with a minimum expense, to achieve the compliance with this new Standard.

Basic fall protection training is now available from several training providers, including corporations' own HR/Safety/Training departments. Our new & unique "Qualified Inspector of Fall Protection Equipment" Seminar was developed for people who already know what harnesses, shock absorbers, horizontal lifelines and other fall arrest system components are. Our Seminar goes one step further and trains participants in inspecting fall protection equipment, and in setting up their own inspection program.

The training method employed by us is the hands-on inspection of real equipment of which the majority was heavily used. The participants will work in small teams (not larger than 4 people), and move from one equipment table to the next. Everybody has his/her sample to inspect and to fill-in the inspection forms. The inspected equipment includes harnesses, lanyards, shock absorbers, fall arresters and lifelines, self-retracting (a.k.a. retractable) lifelines and lanyards, rescue/control descent devices, horizontal lifelines, anchorage attachments and more. The makes come from most major North American manufacturers and suppliers.

This unique, not available from any other training provider, Seminar is an opportunity to enhance employee safety, to comply with regulations, and to save money in the process by learning how to prolong the service life of your equipment the safe way. On the other hand, it assists to remove from use such equipment which is no longer reliable and may fail during fall arrest.


INSPECTION TRAINING - LEGAL REQUIREMENTS

    • Employee shall use
      • Individual Fall Arrest System
      • Fall distance <= 1.2 m
      • Anchor strength >=17.8 kN
      • Removed from service if subjected to fall and inspected by competent person
  2. Province of Newfoundland and Labrador - Fall Protection Regulations CNR 1165/96
    • Safety Belts and Lifelines 104/79 s32 60:
      • (14) Belts, straps, harnesses .........shall be carefully inspected before use
      • (15) If an impairment of function is detected, the defective part shall be removed from service
  3. Province of Nova Scotia
    • Revised Statutes of Nova Scotia. 1989, Chapter 320
      • Fall Arrest Systems:
        • (5) Each component of a fall arrest system.....shall be inspected by a competent person...
        • (6) A fall arrest system that has arrested a fall shall:
          1. be removed from service and inspected by a competent person
